Anyone may make a referral or service inquiry to Origami. Typically, a physician, hospital discharge planner, social worker, case manager, other medical provider, client, or a client’s family member will make the initial contact. However, clients can be admitted from a variety of locations including acute-care hospital, subacute care, LTACH, skilled nursing facility, other types of rehabilitation facilities, or from the client’s home. We recognize this can be an overwhelming experience and our team will make every effort to ensure a smooth admission. We walk families through the admissions process with an action plan that identifies each important next step.
