PROGRAMS
Residential – Transitional (post-acute rehabilitation)
- 15 individuals completed this program in 2005
- Average Length of Stay was 62 Days
- 60% of new admissions were from hospitals; 26% from 24-hour supervision facilities; 7% from home with family support; 7% from home without family support
- 80% of individuals were discharged to independent living or home/family; 13% to AFC home; 7% to community-based living with support
Residential – Long Term (n=6)
- 80% of Long Term residents were active in employment, volunteer, and/or school
- 100% of residents participated in community-based activities averaging 2+ times per week
Day Treatment (n=10)
- Average Length of Service was 4.7 months
Outpatient (n=35)
- Average Length of Service was 4.7 months
Community Integration (n=4)
- 100% of individuals sustained living status for 6+ months
