PROGRAMS
Residential – Transitional (post-acute rehabilitation)
- 19 individuals completed this program in 2007
- Average Length of Stay was 99 Days
- 69% of new admissions were from hospitals and health care providers; 3% from home with family support; 18% case managers
- 24% of individuals were discharged to independent living; 29% to home with family support; 35% to community-based living with support; 6% to 24-hour supervision (Adult Foster Care)
Residential – Long Term (n=8)
- 80% of Long Term residents were active in employment, volunteer, and/or school
- 100% of residents participated in community-based activities averaging 3+ times per week
Day Treatment (n=13)
- Average Length of Service in this program was 2.5 months
Outpatient (n=17)
- Average Length of Service in this program was 7.4 months
Community Integration (n=4)
- 100% of individuals sustained living status for 6+ months
