Origami offers a full range of treatment interventions to support successful outcomes of each individual. For those utilizing more than one service, a holistic approach optimizes the potential for recovery.

Program Admission Criteria

1. Male or Female 18 years or older
2. Requires retraining any of the following Functional Outcome Focus Areas (FOFA):
   
   

   Self-Care	Leisure	Self-Direction/Initiation
   Safety	Productivity	Social Skills
   Learning	Community Access	Independent Living
   Communication

3. Medically stable, free from acute infection or fever
4. Does not require daily skilled nursing
5. Minimum Ranchos Los Amigos Level of 4/5
6. Not a danger to self or others
7. Not actively engaged in substance abuse

Program Focus

• Individualized rehabilitation treatment to reach long term goals
• Ongoing stakeholder education and support

Additional Services

• Group Therapy

Program Discharge Criteria

1. Individual has achieved a level of functional independence and/or long term goals have been achieved necessitating the discontinuation of professional rehabilitation services as deemed by the applicable service and/or treatment team
2. Individual’s behavior is not compatible with the program goals, group activities, or may prevent the progress of other participants*
3. Poses a risk or danger to self or others*

*A discharge procedure will be followed ensuring the opportunity to access other appropriate resources
