       Community Integration Program

The Community Integration Program (CIP) at Origami fosters the generalizing of remedial and compensatory skills learned during residential treatment to a less restrictive environment (i.e. home/apartment, work place, community). In CIP, each person’s rehabilitation goals are specifically tailored for his/her “real-world” setting. This allows for a more seamless integration back into society. - more information -

Program Admission Criteria

  1. Male or Female 18 years or older
  2. May require minimal support or supervision in any of the following Functional Outcomes Focus Areas (FOFA) while residing in the community:

    Self-Care Leisure Self-Direction/Initiation
    Safety Productivity Social Skills
    Learning Community Access Independent Living
    Communication    

  3. Medically stable, free from acute infection or fever
  4. Does not require daily skilled nursing
  5. Not a danger to self or others
  6. Not actively engaged in substance abuse

Program Focus

  • Support community based, semi-independent living
  • Establish and foster maximum independence in all areas of daily living
  • Provide guidance for vocational or avocational goals
  • Facilitate healthy social interaction and involvement in the community
  • Ongoing stakeholder education and support

Levels of Support

  • Transitional Apartment Living (community based)

                   - Base: 1-6 hours of support per week

                   - Standard: 6-10 hours of support per week

                   - Intensive: 10-16 hours of support per week

  • In-Home Support: Rehabilitation Aide as needed

Additional Services

  • Individual Therapy, as needed
  • Group Therapy, as needed
  • Day Treatment, as needed
  • Origami scheduled evening activities, as needed

Program Discharge Criteria

  1. Individual has achieved a level of functional independence necessitating no programmatic support because of systems in place at home and/or work in all 10 FOFA areas
  2. Individual’s behavior is not compatible with the program goals, group activities, or may prevent the progress of other participants*
  3. Poses a risk or danger to self or others *

*A discharge procedure will be followed ensuring the opportunity to access other appropriate resources
