              Semi Independent Living Program

This program is a new building equipped with 6, one bedroom apartment units located on the Origami grounds, a clubhouse, and an administrative wing. This program is designed to meet the needs of individuals who require daily support with advanced activities of daily living either as a transition back to the community or as an extended stay option.

Program Admission Criteria

    1. Male or Female 18 years or older (ages 16-17 may be admitted with special consideration)

    2. Requires 5 hours or less of daily support in any of the following functional areas:

Self-Care
Leisure
Self-Direction/Initiation
Safety
Productivity
Social Skills
Learning
Community Access
Communication
Independent Living
   

    3.  Capable of transferring self with no more than modified independence

    4.  Medically stable, free from acute infection or fever

    5.  Does not require daily skilled nursing

    6.  Can participate in a minimum of 6 hours of programming a day

    7.  Not a danger to self or others

    8.  Not actively engaged in substance abuse

  • Supported, semi independent living
  • Establish and foster maximum independence in all areas of daily living
  • Provide guidance for vocational or avocational goals
  • Facilitate healthy social interaction and involvement in the community
  • Ongoing stakeholder information and support

Additional Services

  • Individual Therapy
  • Group Therapy
  • Scheduled Community Activities

Program Discharge Criteria

  1. Treatment team's assessment indicates that the individual has achieved a functional level of independence requiring less than daily support in the functional areas: Self Care, Safety, and Independent Living
  2. Individual's behavior is not compatible with the program goals, group activities, or prevent the progress of other participants*
  3. Poses a risk or danter to self or others*

 *a discharge procedure will be followed ensuring the opportunity to access other appropriate resources
