Individualized
treatment plans are established upon admission clearly defining
rehabilitation goals and potential discharge plans. All programming
integrates the Hierarchy of Neuropsychological Functions,
a progressive hierarchy of cognitive skills that build upon
each other in a manner that facilitates efficient progress.
The treatment team utilizes the approach to ensure proper
coordination of all therapies.
Program
Admission Criteria
- Male
or Female 18 years or older
- Requires
more than 12 hours of structured support in the Functional
Outcome Focus Area (FOFA): Independent Living
- Requires
therapeutic intervention in any of the following Focus Areas:

- Capable
of transferring self with no more than moderate assistance
- Medically
stable, free from acute infection or fever
- Does
not require daily skilled nursing
- Minimum
Ranchos Los Amigos Level of 4/5
- Can
participate in a minimum of 3 hours of therapy per day
- Not
a danger to self or others
- Not
actively engaged in substance abuse

- Promote
the least restrictive discharge placement
- Provide
guidance in pursuit of vocational and avocational goals
- Facilitate
healthy social interaction and involvement in the community
- Promote
involvement in an integrative trans-disciplinary brain injury
program
- Ongoing
stakeholder education and support

Program
Discharge Criteria
- Individual
has achieved a level of functional independence necessitating
less than 12 hours of support per day
- Treatment
team’s assessment indicates 3-4 consecutive months that
the individual has not progressed in Focus Areas: Self Care, Safety,
and Independent Living
- Individual’s
behavior is not compatible with the program goals, group activities,
or may prevent the progress of other participants*
- Poses a
risk or danger to self or others*
*A discharge
procedure will be followed ensuring the opportunity to access other
appropriate resources
