Make a Referral
Anyone is welcome to make a referral or service inquiry to Origami, including but not limited to:
- Case managers
- Client’s family members
- Hospital discharge planners
- Social workers
- Other medical providers
Individuals can be admitted from a variety of locations including acute-care hospital, subacute care, LTACH, skilled nursing facility, other types of rehabilitation facilities, or from the individual’s home.

We accept funding from many sources, including but not limited to:

- Auto insurance
- Workers compensation
- Private pay
