This program is offered for respite, short term rehab, and long term support needs within the setting of a licensed adult foster care home. Within this program, there are three tracks: Neuro-Rehabilitation, Assisted Living, and Semi-Independent Living.

Short-Term Rehabilitation

Short term rehabilitation focuses on creating the least restrictive discharge plan, participation in intensive therapy services, and significant short term progress in recovery.
